Selecting the box below, I have the authority to commit my child to playing for the Football Club of New Orleans for the 2023/2024 season ending 30 June 2024. I also understand the season can end as early as the end of April 2024. *

Selecting the box below, I am committing to attending a minimum of two training sessions per week held either on Monday, Tuesday, Wednesday and Thursday evenings or if participating in a school based training program a minimum of three weekly training sessions based at school based locations. Evening training sessions are held at Community Park , 815 Community Street, Arabi, La 70032 begin as early as 6:00pm and can end as late as 9:00pm. Matches are normally held on the weekends. Tournaments are weekend long events and can be held outside of New Orleans. *

Selecting the box below, I am committing to actively supporting the club in fundraising activities/events. I further acknowledge the Football Club of New Orleans is a private entity entirely funded through parent fees, events and private fundraising activites. Fundraising goals will be set and expected to be acheived to keep your child in good financial standing. Fundraising activities are designed to directly reduce training, tournament, travel fees and club acquisition of equipment and/or maintenance. A good year of fundraising has seen parents spend as little a $200 in fees for their child to participate in all activities at F.C. New Orleans. *

Selecting the box below, I acknowledge that there are 3 types of game events in which my child will participate in at F.C. New Orleans. The first event is a friendly match in which participation is highly encouraged. The other two events are league and tournament matches. The League and Tournament matches are MANDATORY. The club can occur fines if not able to fully participate in either the League and/or Tournament matches. Full participation is at least 10 players or more are present to play at a League or Tournament Match. Absent players who cause the team to fall below 10 players will be assessed fees to cover cost of forfeits. *

If opting for a payment plan, a deposit must be received by 31 July 2023 to reserve your child's spot on the roster. The annual training fee ($750) is a non-refundable fee once your child participates in one official team practice after you have signed this online form. *

Please be aware that all players must wear only club uniform to matches and club gear to training sessions. If a player does not have club gear, the club will provide a replacement item and the player's family is responsible to pay for the new gear. In addition, there may be request for additional fees for tournaments. Please request a complete fee schedule appropriate for your child's age group for more details. *
